Overview:
Under the direction of the Pharmacy Manager, the RN - Intake Coordinator works in conjunction with referral sources, care coordinators, and/or liaisons to organize, implement, and maintain the intake process for future home care patients. The job includes gathering information that is required for admission of patients into our home care program and creating a smooth workflow for patients to transition into the home care program. The RN - Intake Coordinator job duties also include assisting with patient scheduling as well as being willing/able to provide skilled nursing services to patients as needed and complete other duties as assigned. Perform duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.
Qualifications:
- Education: Graduate of an accredited school of nursing.
- Licensure: Current RN License in applicable state of practice. Current valid driver's license.
- Experience: At least one year previous experience in clinical nursing. More than three years clinical experience preferred.
- Certifications: BLS certification. Current valid driver's license and ability to drive.
- Other: Ability to read and communicate effectively in English. Strong written and verbal skills. Basic computer skills.
